Role of the Case Manager
The case manager is required to take a leadership role in coordinating the integrated case management process.

• Staying in contact with the family;
• Serving as the primary support to the family in gaining access to services; and
• Maintaining connections with other organizations to ensure the client’s care plan is implemented.
